This program serves East Central Minnesota residents with serious and persistent mental illness (SPMI) or serious mental illness (SMI) who are referred by the Adult Mental Health Initiative through targeted case management and a mental health treatment team.

Hours: Monday-Friday 8:00 am - 4:30 pm

Program Description & Details

This program serves East Central Minnesota residents with serious and persistent mental illness (SPMI) or serious mental illness (SMI) who are referred by the Adult Mental Health Initiative through targeted case management and a mental health treatment team.

Primary Service Activities

  • Job exploration and job seeking assistance
  • Individualized plan focusing on recovery and wellness
  • Monthly meetings with employment consultants to help ensure career success
  • Benefits planning
  • Follow-along supports
