Traumatic Brain Injuries

Rise offers day programming and vocational services specifically designed for people who have traumatic or acquired brain injuries, ages 18 and older. Participants benefit from customized supports which help them increase their personal choices, vocational skills, peer relationships, self-esteem, social and community skills, and physical abilities.

People can choose to participate concurrently in these three service areas:

Brain Injury Connection : Exploring Vocational and Employment Opportunities

Brain Injury Connections can assist you in exploring vocational issues and assessing your basic skills, behaviors, and abilities. The program offers evaluation, vocational training and supported employment as well as more specialized kinds of supports and services to help increase your self-dependency and community integration.

Depending on your own needs and interests, you can participate in a wide range of services. These include behavioral assessment and programming, activities leading to more independence, cognitive rehabilitation training, prevocational services, supported employment, career planning, job training, vocational assessment, supported employment, working in Rise’s Integrated Industry, job placement, and follow-up services.

Adult Day Program: Social Outlet

Rise’s Adult Day Program is a social and recreational outlet provided in an accessible setting. It is a place to create friendships, learn new things, and have fun.

You may participate in many different kinds of activities, including discussion and education groups, crafts, cooking, music, games, gardening, creative arts classes, book club, daily exercise, and spirituality groups. You will also have access to therapy services (with doctor’s orders), warm-water pool, and fitness classes/center.

Other services offered through Rise’s Adult Day Program from which you might benefit include personal care assistance, medication administration, support of therapy programs (i.e., walking), individualized service planning, and nutritious meal and snacks.

Structured Day Program: Therapeutic Behavioral Programming

Structured Day Program (SDP) is a small group treatment program which provides therapeutic behavioral programming for those whose cognitive and behavioral issues impact their ability to interact in the community.

SDP provides psychoeducational groups, community skills-building activities, and one-to-one interaction. These services are designed to help you develop good social skills, increase positive behaviors, identify and use compensatory strategies, and develop adaptive coping skills.

You will actively participate in identifying your own treatment goals which are reviewed frequently. Support team members provide consistent positive reinforcement to help ensure your success.

Funding for these programs typically comes from Minnesota Rehab Services, Medicaid waiver programs (TBI or CADI), workers’ compensation, or county social services, or the services are self-paid. Rise staff are knowledgeable in reimbursement systems and will work closely with individuals and families to identify payment options.
