Students in Transition

Graduation from high school is considered a milestone in the lives of most students. When new graduates move into adulthood, they often continue their education or embark on a career of interest.

Rise’s Milestones program serves students from local transition school programs in the Twin Cities area, ages 18-21, who have employment-related goals written in their school IEP (Individual Education Plan). Rise also offers transitional services for students with emotional and intellectual disabilities who are in SCRED, Chisago County and surrounding school districts.

Through an array of comprehensive and individualized services, Rise transition staff can help students prepare for and obtain meaningful, productive employment. By matching a student’s vocational interests, skills, and potential, he or she will more likely enjoy success on the job. Rise can also offer information to resources and referrals to adult programs to address other issues the young adult may be facing.
